REVIVE SAUNA SUITES LLC SERVICE and LIABILITY WAIVER

Thank you for choosing Revive Sauna Suites for your Infrared Sauna/Infrared Light Therapy and Red-Light Therapy needs. Before booking your appointment, please carefully read, review, and agree to the following service and liability policy:

Health Considerations: By checking the Box:

1. You understand that our Infrared Sauna/Infrared Light Therapy and Red-Light Therapy offered are not a substitute for professional medical advice, diagnosis, or treatment.

2. You acknowledge that it’s advised to consult with a healthcare professional before using these services. It is especially encouraged if you have an existing health condition. You also agree that if you have any health concerns or are uncertain whether a medication you are taking may pose contradictions, you must consult with your medical professional, physician, or pharmacist before engaging in these services. You understand that Revive Sauna Suites reserves the right to deny services based on health concerns.

3. You agree to take full responsibility for consulting with your medical professional and refrain from using our services if advised to do so by a medical professional. It is also your responsibility to let Revive Sauna Suites LLC know of any temperature or time limits advised by any medical professional.

If you choose not to seek advice or participate against the recommendation of medical professionals, you do so at your own risk and assume full responsibility for any consequences that may arise.

Medical Contraindications: By checking the Box:

1. You understand that Infrared sauna use is contraindicated for people with the following conditions:

Individuals with an existing fever
People with sensitivity to Heat
Cardiovascular Conditions or problems including hypertension.
Pacemakers
Pregnancy: Consult your physician if you are pregnant.
Children; Seek permission and recommendations from a doctor for any child under 12.
The Elderly - The ability to maintain core body temperature decreases with age. Lower temperatures and time limits are recommended. Consult with a medical professional for permission and restrictions.

Other conditions to consider:

Chronic conditions or diseases associated with a reduced ability to sweat or perspire. These conditions include but are not limited to some autoimmune conditions, Anhidrosis, MS, and Diabetes with Neuropathy.
Some medications, including some over-the-counter drugs - Please check with your healthcare professional or pharmacist to check the drug/medication effect when the body's temp is elevated or its effects that may impair the body's natural heat loss abilities.
Hemophiliacs / Individuals Prone to Bleeding
Joint injury- first 48 hrs.

2. You understand that Red-Light or LED light Therapy use is contraindicated for people with the following conditions:

Sensitivity to light
Epilepsy
Using photo-sensitizing drugs or having migraines brought on by light.

Conduct and Safety: By checking the box you agree:

To conduct yourself respectfully during your visit.
That you comply with all instructions and properly use the facilities and equipment as required to ensure safety.
You will discontinue the use of the sauna if you feel light-headed, dizzy, or heat exhaustion.
You are at least 18 years of age. If under 18 you will provide parental consent.
If you experience pain and/or discomfort while in the sauna or using the LED light panels you will immediately discontinue use.
While using the Red LED light panels; it is your responsibility to wear the eye protection we provide (or you can bring your own.)
Refrain from alcohol use before Sauna use. Alcohol use increases the heart rate, which may be further increased by heat stress.

Liability Release: By checking the box you acknowledge and agree to the following:

That Revive Sauna Suites, its employees, and affiliates are not liable for any injury, discomfort, or adverse reactions resulting from the use of the Infrared Sauna and Red- Light Therapy services. You release Revive Sauna Suites from any, and all liability associated with these services.
By voluntarily participating in the use of our Infrared Sauna and Red-Light devices and entering the premises of Revive Sauna Suites LLC for such usage, you accept any risks associated with their use. You, as well as your heirs, executors, representatives, or assigns, now release Revive Sauna Suites LLC and its owners, officers, and employees, from any claims or liabilities arising in connection with your use of our infrared Saunas and/or Red-Light devices.
This Agreement/Waiver remains in effect for all current and future use of our infrared sauna and/or red-light devices unless specifically requested by either party.
Revive Sauna Suites is not responsible for any damage to personal electronic devices used inside the sauna, or for any lost or stolen items.

Appointment Cancellation and Rescheduling: By checking the box you agree:

A 2-hour notice is required for appointment cancellations and 1 hour for rescheduling.
If canceled two hours prior we will refund your payment minus the Credit card processing fees that we incurred.
No Shows will result in no refund or will count as a used session from your monthly package.

By checking the box, you understand and agree that you have fully read, understood, and agree to abide by the terms and conditions outlined in this Service and Liability Waiver. Thank you for your cooperation.
